GLASGOW, Ky. — Student athletes across the region will have an opportunity to get required athletic clearances at no cost during an upcoming free sports physical event scheduled in Barren County.

T.J. Samson Rehabilitation and Sports Medicine will host free sports physicals on Saturday, May 16, from 7:30 a.m. to 10 a.m. at the T.J. Health Pavilion Rehab Department.

The event provides KHSAA-approved annual pre-participation physicals for student-athletes in grades 5–12, according to a flyer.

Organizers note that the clinic is limited to sports physicals only.

Families are instructed to complete the required KHSAA physical forms in advance, including pages one, two, five and six, which must be filled out by a parent or legal guardian and printed before arrival.

Click here for the high school forms, and here for the middle school forms.

The remaining sections will be completed during the visit, and a parent or guardian signature is required. Students playing on middle and high school teams must complete page 1 from both forms.

Organizers remind students to wear corrective eyewear if needed and note that all documentation must be completed before participation.

The event is free and sponsored by T.J. Samson Rehab and Sports Medicine.
